The forms required to complete a CPHS application are as follows:

• CPHS Application Cover Sheet 
• Protocol Narrative Form
  
• Checklist for Financial Conflict of Interest: Human Subject Studies
• Financial Conflict of Interest Form  (if applicable)
• Protocol Continuation/Renewal Form (if applicable)
• Study Amendment Form (Complete this form if requesting for amendments to protocol.)

You will also need the following Xlab Consent Forms. Please view these approved documents and use them as templates for your own consent form/s. Copy and paste the information into a Word document and change any variable information fields, making them specific to your unique experiment.

• Dowload the <CONSENT FORM: APPENDIX IVA>.
• The <PHOTOGRAPHIC, AUDIO, and/or VIDEO RECORDS RELEASE FORM: APPENDIX IVC> is required if you will be recording your subject via digital or analogue means.
• The <ONLINE CONSENT FORM: APPENDIX IVB> is available for internet-based experiments.
• The <SAMPLE DEBRIEFING FORM: APPENDIX VI> is required when conducting experiments that use deception.
