human subjects: requirements for CPHS:

New, Continuation/Renewal, and Amendment applications for research that fits within the scope of the Xlab Master Protocol are now accepted for review on Berkeley eProtocol. Xlab protocols submitted on Berkeley eProtocol will be pre-reviewed by the Xlab Administrator and then forwarded to Director of the Xlab for review and approval. If your project does not qualify for review under the Master Protocol, you will need to submit a paper application to CPHS for non-exempt review in the old-fashioned way.
Before submitting an application via eProtocol, please consult with Xlab Administrator Brenda Naputi. Email naputi@haas.berkeley.edu or call 510-642-8780.

 

New Research Protocols

STEP ONE: First–time Xlab researchers MUST complete the Human Subjects Training and save a certificate of completion as a PDF document. You will need to attach this to your eProtocol Application.

STEP TWO: Adapt the Xlab Consent Form Templates and Recruitment Materials to your study and save as a PDF document. See templates below.

STEP THREE: Convert all materials that will accompany your application into PDF documents.

STEP FOUR: Log in to <Berkeley eProtocol> using your CalNet ID and create a new protocol.

STEP FIVE: See <eProtocol Quick Guide>: Create a New Xlab Protocol.

 

Continuing Research (Renewals/Continuations)

STEP ONE: Complete the CPHS Study Renewal/Continuation Form and save as a PDF document. You will need to attach this to your eProtocol Application.

STEP TWO: Save your consent forms, recruitment materials, CITI completion certificate and any other materials associated with your protocol as PDF documents.

STEP THREE: Log in to <Berkeley eProtocol> using your CalNet ID.

STEP FOUR: You will need to create a new protocol; however, you will be able to copy and paste most sections from your existing CPHS protocol. See <eProtocol Quick Guide>: Create a New Xlab Protocol.

 

Amendments (Changes to Research)

Note: only amendment application for research previously approved on eProtocol may be submitted via eProtocol. All others must be submitted in hardcopy (see <What to Submit – Amendment>).

STEP ONE: Log in to <Berkeley eProtocol> using your CalNet ID.

STEP TWO: See <eProtocol Quick Guide>: Start an Amendment Application

 

You will also need the following Xlab Consent Forms. Please view these approved documents and use them as templates for your own consent form/s. Copy and paste the information into a Word document and change any variable information fields, making them specific to your unique experiment.
