Researchers using human subjects must adhere to the following procedures:

STEP ONE: First–time Xlab researchers are REQUIRED to complete the Human Subjects Training at <https://www.citiprogram.org/default.asp>
and provide a copy of the certificate of completion to the Xlab.

STEP TWO: If you have CPHS approval already, please provide a copy of the approved CPHS Protocol Narrative Form, the approval letter and the stamped consent form.  (Skip items #3 and #4 if you already have CPHS approval.)

STEP THREE: If you are requesting for expedited CPHS protocol approval you must meet the guidelines of the <XLAB MASTER PROTOCOL>.

Contact Brenda Naputi at <naputi@haas.berkeley.edu> or call 510-642-8780 for an appointment to review your human subjects protocol application. Your human subjects protocol application will be forwarded to the Director of the Xlab for review and approval, and then sent to CPHS for final approval.

STEP FOUR: Please see the <UCB CPHS forms> page to access the forms required to complete a CPHS application.
